Ooma Telo to Cisco FXO config

I have an Ooma Telo that I want to connect to my Cisco CME through an FXO card. I can receive incoming calls no problem. I cannot make outgoing calls. The Ooma seems to work sometimes when set for ground start. Does anyone have this working? What is the config for the FXO port? As soon as I dial “9” to hit the outbound dial-peer, I get a “fast-busy” from the CME. I believe this is because the Telo is not providing line voltage to the FXO port and the FXO does not know the Ooma is connected. When I check the status of the FXO port, it is DORMANT. How do I get the Telo to output line voltage so that the FXO knows it is connected?
