Need Help Porting a Number Out - Horrible Experience

Why the hell is it so hard to port a number out of Ooma? This makes no sense, I called into Ooma and the customer service rep tells me to use the following on my port request.

Name: Ooma Inc <— Why would you use the company name and not my name that is on the account?
Address: 1880 Embarcadero Rd, Palo Alto, CA 94303
Account Number: 1+(Telephone Number)

4 tries in and Ooma keeps rejecting the request stating that they need additional information. I am beyond frustrated at this point. What information do I need to give to my new service provider to get this resolved? Is there anyone at Ooma who can respond to this?
